Job Title: Assistant Director of Nursing- Full Time Exempt
Job Description: The Assistant Director of Nursing is an RN who is responsible for the overall supervision of nursing care in the Health Center.Supervises direct nursing care in the Health Center.Makes routine rounds to assure proper care and a safe, clean resident environment.Assures accuracy and completeness of medical records.Supervises nursing personnel on all shifts. Participates in staffing, scheduling, counseling, orientation, and evaluation of nursing staff.Participates in family/resident counseling.Responds to emergency calls as necessary.Assures effective communication with other shifts and departments as it relates to resident care.Functions in the capacity of DON in his/her absence. Assists DON in developing and implementing policies and procedures.ADON directly reports to DON.Ideal candidate must be RN currently licensed in the State,current CPR certification, geriatric/rehabilitation experience with a good understanding of restorative nursing. Candidate must have minimum 5 years of experience along with some supervisory experience. Some “on call” is required.

Job Title: Certified Nursing Assistant (CNA) - Night Shift
Job Description: Provides direct care to residents in the facility, primarily for their activities of daily living, assistance with therapeutic activities program, restorative programs, documentation as required by the facility.Certified Nurse Assistant (CNA) should be current with State of Illinois certificate.Must be able to read regulations and write clearly.Must be able to communicate in courteous, friendly manner.Ability to read, analyzes, and interrupts human service periodicals, professional journals, technical procedures, and governmental regulations. Ability to write reports. Ability to effectively present information and respond to questions from managers, clients, customers, and the general public.It is a night shift job. MUST BE A CNA (CERTIFIED NURSING ASSISTANT).

Job Title: Community Life Assistant- Part-time
Job Description: Community life Assistant works in accordance with established policies and procedures and /or specific instructions from Director of Community Life. Helps in assisting programs to meet the physical, social, spiritual, intellectual and creative needs of residents. Also responsible for making recommendations for future programming within the community. Coordinate community wide functions to be held. Heads resident program as needed. Assist in creating and posting resident communications, monthly newsletter and calendar. Ideal candidate must have knowledge of gerontology and sensitivity to the leisure interests of the residents that will offer ways to meet their physical, mental and psychological well- being. Must have ability to organize activities and coordinate team efforts. Must be detail oriented and handle multiplicity of tasks. Candidate must posses advanced computer skills with minimum two (2) years of experience in resident activities or related fields.

Job Title: Hospitality Aide- Full Time Non-Exempt
Job Description: Hospitality Aide will assist in food preparation and serving to maintain clean and sanitary conditions in the kitchen and dining areas under the supervision of the Dietary Supervisor/Manager or Cook.Assist in the serving of meals and snacks; use portion control procedures; assist in cleaning and sanitizing work areas, equipment and floors, dishes and utensils; assist in checking trays for menu and diet preferences and accuracy; ensure proper storage of foods and supplies; adhere to universal precautions and sanitary including temperature logs, dishwasher temperatures, and infection control policies and procedures.Assist in the serving of meals and snacks; use portion control procedures; assist in cleaning and sanitizing work areas, equipment and floors, dishes and utensils; assist in checking trays for menu and diet preferences and accuracy; ensure proper storage of foods and supplies; adhere to universal precautions and sanitary including temperature logs, dishwasher temperatures, and infection control policies and procedures. Job Title: IL Housekeeper (Non-Exempt)
Job Description: Under the supervision of the Director of Facilities Management and/or their designee, the Housekeeper is primarily responsible for the daily cleanliness and sanitation of all assigned common areas, hallways,rest rooms, dining rooms, offices and furniture in accordance with federal, state and local standards and guidelines, and as may be directed by the Director of Facilities Management and/or their designee, to assure that the highest degree of quality resident care is maintained at all times.Must possess general knowledge of cleaning chemicals, equipment and procedures, infection control procedures and universal precautions. Must be able to read, write and follow written and verbal directions.High School education or GED preferred, but not required.

Job Title: LPN - Day Shift (Non- Exempt)
Job Description: Licensed Practical Nurse will provide direct nursing care to residents and delegates and supervises the care provided by nursing assistants. Administers medications, performs treatments and procedures, obtains specimens as needed, completes documentation as required.Candidate must have Licensed Practical Nursing degree with a current State of Illinois license. Two years of experience working as a licensed practical nurse including some medical/surgical experience and awareness of the needs of geriatric residents.Must be able to read regulations and write clearly, and also communicate in courteous, friendly manner. This is a day shift position.

Job Title: Registered Nurse (RN) - Night Shift
Job Description: Supervise and provide quality nursing care to meet residents needs,coordinate and conduct activities of daily living, to address family/social concerns, document and contribute to health status assessments and care plans, administer medications, and performs skilled treatments and procedures.Registered Nurse (RN) should be current State of IL license.Must be able to read regulations and write clearly.Must be able to communicate in courteous, friendly manner.Ability to effectively manage and direct subordinates.Registered Nurse (RN) should have minimum Associates Degree in Nursing.Two (2) years of experience working as a registered nurse including some medical/surgical experience and awareness of the needs of geriatric residents.It is a night shift job. MUST BE A RN (REGISTERED NURSE).

Job Title: Resident Aide-Weekend Day Shift
Job Description: The Resident Aide provides personal care and non personal care assistance to residents in our memory care (Willow Place) unit. Assists residents so that memory care living can be maintained. Promotes memory care living activities while giving assistance to the residents. Resident Aide helps memory care unit residents to meet their physical, emotional, and psychosocial needs.Ideal candidate should have 2 years of work experience.Must have ability to care for and work effectively with memory care residents.This position is every weekend day time shift.

Job Title: Restorative CNA - Full Time

Job Title: Restorative Registered Nurse- Full Time (Day Shift)
Job Description: Restorative Registered Nurse will manage the Restorative Therapy Nursin department. Plans, develops, organizes, implements, evaluates and directs restorative therapy services for residents. Educates facility staff in restorative therapy and its execution. Maintains complete and accurate records of therapy service assessments and services. Coordinates restorative therapy services with therapy service provider to provide transitional services to facility residents. Compiles reports related to restorative therapy and resident safety. Candidate must be certified Restorative RN and should be current with State of IL license.Must be able to read regulations and write clearly.Must be able to communicate in courteous, friendly manner.Ability to effectively manage and direct subordinates. Minimum One (1)year of experience working as a Restorative Registered Nurse is required including some medical/surgical experience and awareness of the needs of geriatric residents.It is a Full Time Monday to Friday Day Shift job.

Job Title: Utility Worker- Full Time
Job Description: Under the supervision of the Sous Chef and/or their designee, the utility worker is to perform all duties to ensure the dining facilities are clean and sanitized. Follow all daily and weekly cleaning schedules; perform all proper cleaning and any preventive maintenance of equipment (deliming of dish machine). Performs duties in accordance with local and state health requirements. Cooperates and coordinates work activities with co-workers to ensure a smooth run of the department. Work exeprience should be at least one (1) year as utility worker.
